Prophylactic administration of folic acid should be undertaken only when there is a clear indication. Dietary supplementation is necessary when there is an increased requirement that is not satisfied by normal dietary sources. Pregnancy with the increased demands of the fetus, or lactation, where as much as 50 pg of folate is lost such day in the breast milk, is an indication for supplementation with folate.
